Nursing care: Initial assessment and care plan

The Care Plan sets out the provision of nursing or care support that we agree with you. The cost will relate to a number of factors that include the frequency and duration of care; the type of care required, whether by a qualified nurse or care assistant, for example; and whether care is provided during the day, at night or over the course of a weekend
