GPs - How to refer patients for MRI scans
As a result of NHS reforms, GPs can now refer patients directly for treatments such as an MRI scan. In accordance with MHDA guidelines, to ensure patient safety Vista only accepts referrals from recognised clinicians. The
referral form is designed to obtain the appropriate clinical information and region to be scanned, as well as relevant patient and referring clinician contact information.
To ensure it is appropriate for a patient to undergo an MRI scan a completed Referral Form is required from a referring clinician such as a Consultant, GP, Physiotherapist, Osteopath, and Chiropractitioner. If a patient is not currently under the care of a referring clinician they should contact Vista so that an Independent Medical Assessment of the appropriateness of an MRI scan can be arranged.

Print a copy, complete all sections and post it to them
Or, if you prefer, fax it to them at 0845 450 2171.
Please include as much information as possible regarding the patient’s condition and clearly state the patient’s contact details so that an appointment can be arranged at a convenient time for you and the patient. If you wish a copy of the diagnostic report to be sent to another recipient, such as a consultant, please provide their details.
