Prostate cancer treatments: Radical prostatectomy in the UK

Radical prostatectomy involves removing the entire prostate and seminal vesicles and is considered by many urologists and oncologists to be the 'gold standard' as it allows complete removal of the tumour when organ-confined, as well as definitive staging.

Additionally, it allows absolute confidence in interpreting the PSA level after treatment: a zero level confirms complete eradication of prostate cancer. In the small percentage (7%) of men in whom cancer recurs, it is detectable by a rise in the PSA level 3-5 years before it otherwise would cause symptoms, allowing radiotherapy to be administered. (Surgery also corrects obstruction to the drainage of urine from the bladder caused by enlargement of the prostate.)

 

The procedure

You will be admitted either the day before or on the day of your operation and will be visited by an anaesthetist and a surgeon before the operation to answer any remaining questions. Laparoscopic radical prostatectomy is a complex operation and takes 2-3 hours to perform through five 5-10 mm incisions. 

 

Alternative prostate cancer treatment

For patients who are not suitable for surgery, the use of radiotherapy and observation, or, cryotherapy and ultrasound, are viable therapeutic options.

      

Recovering after radical prostatectomy

You should be able to go home with a catheter after 2-3 days and are re-admitted after the 9th day for catheter removal. You should expect to return to most activities about 3 weeks after laparoscopic and 3 months after open surgery.

The first outpatient appointment will be 4 weeks after the operation, when the results of the laboratory analysis (histology) of your prostate should be available. Your first PSA test will be done 2 weeks before your next outpatient appointment (3 months following surgery). You will be seen every 3 months for a year, every 6 months for 5 years and then annually until 15 years.
