Hand and wrist surgery available

The hand and wrist clinic is run by a very experienced hand surgeon. Treatments offered include:

 

Carpal tunnel syndrome

Carpal tunnel syndrome occurs when there is excessive pressure on one of the nerves at the wrist. Patients complain of numbness and tingling in the fingers and thumb, sleep disturbance, dropping things and having difficulty doing up buttons. There may also be discomfort in the hand, forearm and even the shoulder. It may affect both hands.

 

Treatment ranges from avoiding activities which make the problem worse, wearing a splint at night time, injection of steroid into the carpal tunnel. The latter is an operation which takes less than 30 minutes and usually is performed as a day case under local anaesthetic. When the local anaesthetic wears off there may be discomfort but this does vary from person to person and you will be provided with painkilling tablets as necessary.

 

You can expect to have a fairly bulky bandage applied after surgery which may be reduced 3 days later, otherwise it should be left on until your stitches are removed 10-14 days after your operation.

Dupuytren’s contracture

Dupuytren’s contracture is a thickening of the gristle which forms a protective layer in the palm of the hand. Initially nodules can be felt in the palm of the hand which can progress and thick bands are formed which pull on the fingers so they cannot be fully straightened. Once the fingers are affected to the extent that it is hindering use of the hand then an operation should be considered. The operation is performed under general anaesthetic either as a day case or with just an overnight stay in hospital. The size and shape of the incisions in the palm depends on the extent of the disease. The thickened tissue is cut out and the wound is usually stitched.

 

Sometimes the wound is left open rather than stitching and therefore needs 3-4 weeks to heal over. Very occasionally a skin graft is taken from the upper arm if needed, otherwise the operated fingers are immobilised for 2 days in a boxing glove type bandage and then left free with a protective dressing for about 7 days. The operation is surprisingly painless although simple painkillers may be required.

Surgery to the trapezium

 

 

The trapezium is one of the largest bones in the wrist and lies at the base of the thumb. The joint between the two is the commonest joint in the hand to be affected by arthritis. It is a slowly progressive condition that leads to pain, increasing stiffness and deformity in the thumb. Although there are various treatments for relieving discomfort, surgery is the only definitive treatment for persistent and severe symptoms.

 

 

 

 

 

 

 

 

 

The procedure is performed under a general anaesthetic and requires an overnight stay in hospital. The arthritic bone (trapezium) is removed leaving a space into which a temporary wire will be inserted in order to hold and stabilise the thumb. The skin will be sutured and the hand immobilised in a Plaster of Paris.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

You will need to be in plaster for a total of six weeks during which the hand will need to be elevated in a sling for a week or two following surgery to minimise any swelling.
