The most important advantage of self-monitoring, however, is that the coagulation status can be regularly and frequently checked, so that anticoagulant therapy can be adjusted when necessary. Regular testing is essential because factors such as drugs, illness, nutritional intake, alcohol, travelling, and stress may interfere with one's coagulation status.
Self-monitoring with the CoaguChek® XS systemmeans that anticoagulant therapy can be constantly optimised, which decreases the possibility of complications, such as clots or bleedings3.
INR Self-monitoring: choose independence
Has your doctor just told you that from now on you need to use oral anticoagulant drugs, and that the clotting times of your blood will have to be checked regularly?
Oral anticoagulation therapy is necessary if you have an artificial heart valve, or if you are affected by atrial fibrillation or thrombotic diseases, such as DVT or pulmonary embolism. Patient self-monitoring may also be a suitable option for someone who has suffered heart attack or stroke and is on long term warfarin. Taking an oral anticoagulant (e.g warfarin) is very important for your health, and it is important for the dosage to be exactly right. To achieve this accuracy, regular monitoring is necessary as people react differently to those drugs or as several factors may interfere with it e.g. food, other drugs, etc.
Once you've been released from the hospital this monitoring usually takes place at your doctor's practice or in an anticoagulation clinic. It traditionally requires a blood sample being drawn from a vein. It is common for people to experience long waiting times and potential time losses i.e different appointments, which reduce your independence.
However, recent developments are now allowing patients to monitor medication themselves. This is called coagulation self-monitoring. People who self-monitor usually use a portable monitor, often similar to that used in their clinic, to take a a pin prick of blood from their finger at times agreed with their healthcare professional and record INR measurement, provided by their monitor.
What is the difference between patient self-testing (PST) and patient self-management (PSM)?
People who are self-testing provide the INR reading they obtain from their monitor to their healthcare professional at an agreed time, generally by telephone. The healthcare professional decides if the warfarin dose needs to be adjusted and lets the person know what they should do.
People who self-manage adjust their warfarin dosage themselves, following training with their healthcare professional. This means that if the INR reading obtained from their monitor is out of their normal range, they are able to make an adjustment to their warfarin dose themselves.
Self-monitoring has many advantages: it is straightforward, it makes routine testing easier, it gives you your independence back, and it means you can be directly involved in your own health, collaborating with your chosen healthcare professional. More importantly, experience has shown that more frequent testing reduces complications that result from the underlying disease or from the anticoagulant itself.

Coaguchek® XS Connect
The CoaguChek® XS Connect device is a small, compact, and simple instrument for downloading INR results from a CoaguChek® XS system to a computer for easy interpretation and action. When connected to a PC, the CoaguChek® XS Connect device is recognised by the computer as a mass storage device (MSD) i.e. a drive with removable disk (similar to a USB memory stick). All information from the uploaded devices, but also the elements needed for the user interface appear as files on this drive and are not transferred to the hard disk of the computer.
